Checklist of Documentary Requirements to file a Pension Plan Benefits Claim:
☑ Fully Accomplished Plan Benefit Claims Form (Download PDF)
☑ Photocopy (Xerox) of Pension Plan Agreement (Policy)
☑ Photocopy of Certificate of Full Payment (If Available)
☑ Photocopy of 2 Valid IDs of Planholder with 3 Specimen Signatures
If the Planholder is Deceased:
☑ Photocopy of Death Certificate of Planholder
☑ Photocopy of 2 Valid IDs of Beneficiary with 3 Specimen Signatures
Other Documents to Submit:
☑ If Original Pension Plan Agreement is Lost or Damaged: Notarized Affidavit of Loss & Indemnity Agreement
☑ If Claimant is a Representative of Planholder:
- Notarized Special Power of Attorney
- 2 Valid IDs of Representative
☑ If Planholder is a Minor: Notarized Affidavit of Guardianship
☑ If Claimant/Beneficiary is more than one: Notarized Waiver of Rights
