STEP 1:
Login to your MIS account.
STEP 2:
From the Dashboard, click APPLY e-LOA.
STEP 3:
Enter the last 7 digits of your Policy Number. Click Ok.
STEP 4:
Kindly check if you have entered your Policy Number correctly then click Confirm.
STEP 5:
Select a Hospital or Clinic on the list.
STEP 6:
Select the doctor's Specialization on the list.
STEP 7:
Select a Doctor on the list.
STEP 8:
Review the Doctor's Schedule. Click Submit.
  • The Doctor's Schedule is subject to change without prior notice.

  • Please contact your provider of choice for confirmation of schedule or appointment prior to visit.
STEP 9:
Click Yes to proceed.
STEP 10:
Check the generated e-LOA at the "Downloads" folder of your computer. Your e-LOA is password protected.

REMINDERS
BEFORE AVAILMENT:
  • Membership must be in ACTIVE STATUS.
  • The NOTICE FOR AVAILMENT is timely and the Letter of Authorization (LOA) has been duly secured from CHS before any availment, unless in case of an emergency.
  • Disclose a known pre-existing illness or injury in the Membership Application Form (MAF).
  • Health care service/s must be sought ONLY from an ACCREDITED PROVIDER.
OTHER REMINDERS:
  • Once the LOA has been approved, contact the accredited provider for the appointment details such as schedule and location.
  • LOA is valid up to 12 days only.
  • For cancellation, redating, or lapsed LOA, please surrender or email the unused LOA to the nearest branch.
  • For subsequent availments (elective procedures), bring a copy of request or send a picture or scanned copy of the laboratory examination request with the following details to the nearest branch of choice:
  1. Name of the Member
  2. Policy Number
  3. Birthdate
  4. Contact Number
  5. Chief Complaint
  6. Hospital/Diagnostic Clinic
