CARITAS HEALTH SHIELD, INC. (CHSI) is a duly registered corporation in the Philippines which is one of the established leaders in the field of health care management. Your privacy is very important to us. We want to assure you that CHSI holds all your personal information in the strictest confidence. We are committed to safeguarding your personal information pursuant to the provisions of the Data Privacy Act of 2012 (RA 10173).

CHSI as the Personal Information Controller (PIC) has mandated its Data Protection Officer (DPO) who ensures adherence to the Data Privacy Act, its implementing rules and regulations, and other relevant issuances of the National Privacy Commission. Why do we collect information from you?

CHSI only uses the personal information collected solely for the following purposes:

• Process your application, amendments, reinstatements, collection, and claims;
• Inform you of your payment due dates, lapse dates and other reminders that we believe will be beneficial for you;
• Assist you in services you avail from us;
• Process your complaints and feedbacks;
• Internal operations;
• Product development, service fulfilment and marketing purposes
• Prevention of fraud and other harm;
• Legal compliance, and;
• Ultimately, serve you better by providing a smooth, safe, efficient, and customized experience.

Unless lawfully required and necessary for the tasks at hand, CHSI does not sell, rent, intentionally release, or share your personal information to third (3rd) parties or any other organization. When we share your personal information, we only disclose those that are necessary for them to carry out their functions and we require them to enter into an agreement with us to ensure that only the highest security controls are implemented in safeguarding your personal information. Also, we make sure that no personal information is sold or shared to parties other than CHSI.

How do we collect your information?

We collect your personal information when you login to our website and mobile applications; when you send an inquiry regarding our products and services; when you request for assistance during medical availment; when you use our online proposal system, and when you fill out the Membership Application Forms and submit other documentary requirements.

Our website uses cookies merely to collect statistical data regarding your visit and page preferences. A “cookie” is a small piece of data sent from a website and stored on the user's computer by the user's web browser while the user is browsing.

Risks pertaining to Third Party Links

CHSI website may contain links to other websites. Once data subjects click these links to leave our site, data subjects should be aware that we do not have any control over those websites. Therefore, we cannot be held responsible for the security and privacy of any personal information which data subjects have provided while visiting other sites.

What personal information do we collect from you?

The personal information that we collect to identify or contact you may include your full name, IP address (Internet Protocol), address, age, email address, telephone/cellphone number, birthdate, name of your dependents (e.g., spouse, siblings, etc.,) government ID numbers (e.g., TIN, SSS, Pag-Ibig, etc.,) ethnicity, sex, education, marital status, color, medical history, and other information as defined in RA10173. These information are necessary to address your concerns individually, provide HMO services, process your request(s), conduct surveys, or inform you of special product offers.

How do we transmit your data and where do we store it?

CHSI uses layers of security protection to your personal information. Our website and mobile applications use secured port (https) which employs 256-bit encryption transmission.

Your personal information is housed in our Data Center located in Quezon City, Philippines with at least 4 layers of security (e.g., building perimeter security, building door RFID entry, biometric entrance to data center, password access to server/clients, password access to database.)

How long are we keeping your data and disposal of your personal information?

CHSI will retain your personal information in our systems for five (5) years reckoned from the last transaction date, closure of account, after the release of your maturity benefits or end of contract, provided that, there are no pending case or advise/instruction from any government agency/ies to hold or retain such personal information for a longer period. As required by existing laws, CHSI will dispose or discard your personal information in a secure manner that would prevent further processing, unauthorized access, or disclosure to any other party, or prejudice your interest.

What are your data privacy rights?

As part of recognizing your rights, you may request for the removal/restriction of whatever personal information you provided us. You have the right to access or update your personal information subject to exceptions specified by law. You can review or update any of the information you have given us by filling out our amendment form. Our Customer Relations Services will be glad to assist you in updating your information.
